• Once you are on the site, use the “Eye Care Professional Locator” to find an Eye Care Professional near you.
• Once you have found a local Eye Care Provider, call, and make an appointment for an eye exam.
• If you have a current prescription, less than two years old you do not need a new eye exam. You can simply stop in once the below steps are completed and bring your current prescription. A call to the Eye Care Provider is always a best practice so they can be prepared for your arrival.
• Click on the link "Ordering Safety Eyewear."
• Review the available Prescription Safety Eyewear Packages, there are 3 packages available.
• Important: All orders for this program are paid with a credit card at the time of order. To provide protection, you must first encrypt your credit card information. To protect your credit card and encrypt select the “Click here” link and complete the form to secure your card information. You will receive a Secure Credit Card ID# (SCCID) via email address provided. This SCCID number will be used to make your purchase. It acts like a credit card number without using your actual card number, thus protecting your information.
• Select your desired Prescription Safety Eyewear Package from the drop-down menu.
• Complete the remaining fields, be sure to use your Secure Credit Card ID#.
• Click “Continue” and follow the instructions to print your SRX Voucher Form.
• Take your Form to your identified Eye Care Provider and select your prescription safety eyewear. Remember, you have already paid for your Prescription Safety Eyewear through the portal. The form you printed outlines what is available within the selected package. Your Eye Care Provider will be able to walk you through your options. All prescription eyewear is non-refundable after the product has been made and comes with a one-year, normal wear warranty.
